Anyone can refer someone to LIFT — whether you’re a concerned friend, family member, probation officer, or the individual themselves. If you know someone in Mesa County struggling with mental illness, addiction, or any other complex personal or social challenges, this is the place to get them real support.
If you’re a licensed healthcare provider and plan to share protected health information, please also complete a Release of Information (ROI) form to ensure we can legally review and act on your referral.
The LIFT referral phone is staffed Monday through Friday from 8:00 a.m. to 5:00 p.m. All referrals must be submitted through the official referral system to ensure that all pertinent information is accurately captured and that all applicable Colorado legal requirements are met. Referral decisions will be made within 24 hours of submission, and initial contact with the referred individual will occur within 48 hours.
