What is LIFT?

LIFT (Learn. Identify. Foster. Transform.) is a Mesa County-based program that connects individuals to intensive case management and peer support services through a unified referral system.

Referrals are open to individuals in Mesa County who are living with a substance use or mental health disorder. A payor source is required to access services, and individuals must meet clinical criteria for a behavioral health diagnosis.

Peer Support Services connect individuals with trained peers who have lived experience with mental health and/or substance use challenges. These certified peer specialists use their recovery experience, specialized training, and supervision to promote hope, support recovery goals, assist with navigating behavioral health systems, and foster wellness in everyday life.

Case Managers are trained professionals who coordinate and link individuals to essential services such as food, housing, healthcare, behavioral health treatment, and other community-based resources. In accordance with Colorado Medicaid guidelines, they assess individual needs, develop service plans, monitor progress, and advocate on behalf of clients to ensure access to appropriate and timely care.

There is no cost to submit a referral to the LIFT program; however, a payor source is required for the referred individual to be accepted into services.

The LIFT referral phone is staffed Monday through Friday from 8:00 a.m. to 5:00 p.m. Referral decisions will be made within 24 hours of submission, and initial contact with the referred individual will occur within 48 hours.
